Mail Order Form

You should be able to print our form, fill it out and mail it in with your payment.  We accept personal checks, money orders (made out to QuickBurst), Master Card, Visa and Discover.  We also accept PayPal, please send to sales@quickburst.net.  If you provide an e-mail address we will send you a shipping notice with delivery confirmation information.

 

Sometimes shipping can be a little difficult to calculate, it depends on what you order, the size and weight of the box.  If you have any questions about the shipping charges, please send an e-mail for a quote info@quickburst.net , this is often the best route.  Another option is to ask for a PayPal invoice, we can do this and send it on to you, if you provide your PayPal address.

 

How to use the form:

 

  1. Print the form.
  2. List the products you wish to order in the “Item” column.  Please print legibly.
  3. Indicate the quantity needed in the “Quantity” column.
  4. Indicate sale price in “Price Each” column.
  5. Multiply “Quantity” needed by “Price Each” and enter results in the “Total” column.
  6. Add up the numbers in the “Total” column, this is your purchase price.
  7. Record this total in the “Sub Total” column.
  8. Enter shipping charges in “Shipping charges column (Use the shopping cart to calculate).
  9. Add the columns, and enter in the “Grand total” column.
  10. Send completed form with payment to:

Shipping Charges Explained

Shipping is based on box size, weight and delivery method.  There can be many variables, too many to address here.  Please contact me if you have any questions.  The charges listed below are for CONUS.  If you are ordering from outside the USA, contact us for a quote your charges may vary.  We do ship some products internationally.

 

Use the shopping cart to calculate shipping.  Simply input the items you wish to order then let the cart do the work.  When finished record the shippin and clear the cart.  Easy as pie.
